چکیده

The patient who is profoundly neutropenic and expected to remain so for an extended period of time is at high risk for a serious bacterial or fungal infection.[1,2] In approximately one third of episodes of fever and neutropenia (absolute neutrophil count <500 cells/mm>6), a diagnosis is documented, and of these, a small proportion is life threatening.[2,3] Successful therapy for documented infections is predicated on recovery from neutropenia and the efficacy of available antimicrobial agents. The morbidity and mortality are higher in patients with neutropenia that persists for more than a few days and results in progressive infection, as defined by lack of response to antimicrobial therapy.[2,4]

Granulocyte transfusions in hematologic malignancy patients with invasive pulmonary aspergillosis: outcomes and complications.

BACKGROUND Granulocyte transfusions (GTXs) have been used successfully as an adjunctive treatment option for invasive infections in some neutropenic patients with underlying hematologic malignancy (HM). PATIENTS AND METHODS We sought to determine the impact of GTX as an adjunct to antifungal therapy in 128 patients with HM and prolonged neutropenia (≥14 days) with a proven or probable invasiv...

Granulocyte transfusions for preventing infections in people with neutropenia or neutrophil dysfunction.

BACKGROUND Despite modern antimicrobials and supportive therapy, bacterial and fungal infections are still major complications in people with prolonged disease-related or therapy-related neutropenia. Since the late 1990s there has been increasing demand for donated granulocyte transfusions to treat or prevent severe infections in people who lack their own functional granulocytes. This is an upd...

Effectiveness of Granulocyte Transfusions in Neutropenic Adult Oncology Patients: A Comprehensive Review of the Literature

Among patients with cancer, many factors can cause severe and persistent neutropenia, leading to increased morbidity and mortality. For patients with neutrophil deficiency, replacement with granulocyte transfusion (GTX) seems a rational approach. However, existing data on the efficacy of GTX have been inconclusive, and such adverse effects as respiratory distress and death indicate the need for...

Granulocyte transfusions in severe aplastic anemia: an eleven-year experience.

BACKGROUND Infections, particularly those caused by invasive fungi, are a major cause of death in patients with severe aplastic anemia. The purpose of this study was to analyze our experience with granulocyte transfusions in such patients. DESIGN AND METHODS A retrospective analysis was performed on all patients with severe aplastic anemia who had received granulocyte transfusions between 199...

Granulocyte colony-stimulating factor-primed leukocyte transfusions in candida tropicalis fungemia in neutropenic patients.

Optimal management of fungemia in neutropenic patients is still controversial. Several reports have already stressed the poor prognosis in invasive candidiasis (80% mortality in several reports). Therefore granulocyte transfusions would appear to be useful in the management of these infections. We report the use of rhG-CSF-primed granulocyte transfusions plus amphotericin B in two neutropenic p...
